Four Seasons is continuing to grow and we are looking for Registered Nurses who are positive, motivated and confident in their skills to help provide compassionate end of life care.
Four Seasons continues to exceed our growth projections and require the recruitment of new staff to meet the needs of the growing Agency.
The Hospice Case Manager's responsibilities include:
- performing visits to the patient in their home setting.
- visits include: admission, routine, and death.
- discuss the patients current state and needs with the IDT
- identify patient/family needs and create the Aide Plan of Care
- Develop the Patient Plan of Care
The Registered Nurse is also a part of the Interdisciplinary team and works in conjunction with the other IDT members to provide quality care with the team care module. Four Seasons Healthcare of Texas is committed to providing exceptional end of life care to those when they need it most.
What You Must Have:
- Current State of Texas Registered Nurse License
- Current Driver's License & Reliable Transportation
