Registered Nurse Case Manager (Rn)

We are looking for an outstanding Registered Nurse (RN) to join our team in Houston, TX and the Surrounding Areas..

Core Responsibilities:

• Conduct assessments of incoming patients
• Manage your own case load and conduct visits where patient resides
• Provide symptom management and oversee patient’s plan or care while collaborating with Hospice team members
• Participate in anon-call rotation for evenings and weekends
• Have knowledge and understanding of all services hospice provides and keep patient and family informed of all benefits available

Day to Day Activities:

• Educate patient and family on disease progression, make visits, care coordination and management of patient and family needs related to Hospice diagnosis
• Deliver outstanding bedside care
• Foster relationships with your patients and families to make a difference in their greatest time of need

Who the Job reports to:

• Director of Clinical Services

Requirements/ Qualifications:

• Valid Registered Nurse (RN) license
• 1-2 years of nursing experience (preferred)
• Driver’s license, insurance and vehicle
• Utmost compassion and integrity
• Bilingual (English & Spanish)
