Join the hospital trusted by Chicago's Southland, Ingalls Memorial Hospital, in this role as an Advanced Practice Nurse (APRN) Oncology Navigatorfor our cancer program.
The Advanced Practice Oncology Nurse Navigator works collaboratively with an interdisciplinary team and is responsible for the coordination of care of patients throughout the disease process acting as a clinical liaison for patients across medical services. The Oncology Nurse Navigator provides cancer genetics risk screening and referral for testing; functions as the cancer survivorship coordinator and provides navigation clinical support and supervisory oversight to the oncology navigation social service worker and the genetics data specialist.
Essential Job Functions
  • Survivorship: Functions as survivorship services coordinator by providing oversight for the cancer survivorship team and reporting annually to the cancer committee on survivorship program activities.
  • Genetics: Identify high risk individuals appropriate for cancer genetics evaluation through screening or by referral and assist families with referrals for cascade testing for pathogenic variants.
  • Provide navigation services to patients to minimize barriers to care, provide support and link patients and their families to resources to be able to complete planned cancer course.
  • Completes an assessment of patient needs, barriers to care and coordinates access to services to support patients throughout the cancer trajectory.
  • Completes survivorship care plans and reviews with patients.
  • Supervise and provide clinical support to the oncology navigation social service worker and cancer genetics data specialist.
  • Develop patient and community education programs and tools and provide support for community outreach activities
  • Participate in site-specific, cancer conferences/tumor boards, program development teams and Cancer Committee. Maintain updated knowledge base of guidelines to ensure best practices are maintained for cancer survivorship, cancer genetics and other oncology care standards.

Required Qualifications
  • MS in Nursing, MSP or DNP degree
  • Current Illinois license as a Regstered Nurse (RN), Advanced Practice Registered Nurse (APRN) preferred.
  • AOCNP or OCN (Or equivalent certification from the ONCC)
  • 3-5 years experience in oncology nursing, 5+ preferred
  • Current BLS certification
  • Excellent interpersonal and leadership skills
  • Ability to travel to oncology office locations within the UChicago Ingalls Memorial Care Centers

Preferred Qualifications
  • Specialized education in cancer genetics and hereditary cancer predisposition syndromes
  • Experience in cancer survivorship care
  • Advanced Genetics Nursing Certification (AGN-BC) or Advanced Clinical Genomes Nurse (AGCN) credentials from the Nurse Portfolio Credentialing Commission (NPCC)

Must comply with Ingalls Memorial Hospital's COVID-19 Vaccination requirement as a condition of employment.
