The Qualified Professional is an individual who is a graduate from a college or university with a baccalaureate degree in the field a human service field and has two years of full-time with appropriate age and population, post-bachelor’s degree accumulated Mental Health, Developmental Disabilities and Substance Abuse experience with the population. Applicant that is familiar with Therap and Calcium calendars is a plus. Hours are flexible.
An Associate Professional with experience will also be considered.
The Qualified Professional will be hired, trained and supervised by the Program Director. He or she will directly be responsible for performing the following activities:
v Develop Person Centered Plan and Short Term Goals
v Implementation of the goals developed in the Person Centered Plan and/or ISP
v Supervising all direct care staff and/or I/DD Workers assigned
v Complete all intake and referral documentation
v Coordination of medical, therapeutic and psychiatric care
v Incident Review
v Documentation
v Aid in providing a therapeutic environment
Responsibilities:
When a Qualified Professional is performing in the capacity of a direct care staff the responsibilities are as follows:
1. Provide assistance with therapeutic interventions to rehabilitate functional skills, adaptation , socialization, relational, coping skills, self-management of symptoms, daily and community living skills and behavior and anger management skills.
2. Therapeutic mentoring that directly increases the acquisition of skills needed to accomplish the goals of the Person Centered Plan.
3. Provide psychoeducation and training and training of family, unpaid caregivers, and other s who have a legitimate role in addressing the needs identified in the Person Centered Plan.
4. Direct preventive and therapeutic interventions that will assist with skill building
5. Relapse prevention and disease management strategies
6. Ongoing symptom monitoring and management
7. Ongoing medication monitoring, with report to medical providers
8. Service coordination activities within the established Person Centered Plan/ISP
9. Input into the Person Centered Plan/ISP modifications
10. Provide accurate and clinically written documentation
11. Report and document any accidents and/or incidents to Director
12. Administer CPR/First Aid when needed
13. Follow OSHA guidelines
14. Ensure the self-respect, personal dignity, rights and physical safety of all consumers served
15. Work cooperatively with all HRH, Inc. staff
16. Be familiar with and observe the confidentiality of consumers (HIPAA).
17. Be familiar with consumer rights.
18. Be familiar with HRH, Inc. policy and procedure.
19. Participate in all required training activities.

Pay: $17.12 - $25.00 per hour
