HealthCare Support is actively seeking a Hybrid RN Nurse Evaluatorto fill an opening inNew York, NY.
Daily Responsibilities for Hybrid RN Nurse Evaluator

• Develop Home and Community Based Services (HCBS) Plan of Care (POC) in collaboration with child and family that includes evaluation of health status
• Perform evaluations of children that includes evaluation of health status, strengths, functional care needs, and preferences and guide the development of individualized long-term care service plans
• Retrieve and review child's medical documentation and or health referral forms, as relevant to the case
• Utilize the State CANS assessment tool and MAXIMUS case management systems to document the child's health status, strengths, functional care needs, diagnoses and preferences and transmit, as required
• Shift: Monday to Friday 8am-5:00pm/Hybrid(will spend some of their time working from home on cases and another, equal portion of their time meeting with their clients and their families)

Required Qualifications forHybrid RN Nurse Evaluator:

• Licensed RN in the state of NY
• Experience working with some form of Face to face or over the phone assessment system (OASIS, for example)
• 2 years year of experience in any of homecare/psychiatric/behavioral,health/pediatrics/adolescent/foster care settings/group home/school nurse/care management
• Computer proficiency

Pay at $35 to $46/hr
