Job Responsibilities
Purpose of the Position: This part-time position works to promote and protect the health and safety of the residents of Cobb and Douglas Counties. Under general supervision, promotes, maintains, and improves individual and Community Health by assisting individuals and their families to connect to critical health resources. Collects and analyzes data to identify community needs prior to planning, implementing, monitoring, and evaluating programs/events designed to encourage healthy lifestyles, promote public health policies and environments. Responsible for assisting residents with navigating the healthcare and social services system. Serves as a Community Health Worker. Provides support to increase equitable access to care. Partners internally and externally to meet residents' healthcare needs. Two plus (2+) year grant-funded position (2023-2026).This position works to promote and protect the health and safety of the residents of Cobb and Douglas Counties.
Essential Functions (include, but are not limited to):
Minimum Qualifications
Minimum Qualifications: Master's degree in public health education from an accredited college or university OR Bachelor's degree in education, social, behavioral or biological sciences or a directly related field from an accredited college or university AND one year of experience in public health education.
Preferred Qualifications: Excellent verbal and written communication skills, familiarity with Cobb and Douglas Counties, experience with community outreach, and ability to speak Spanish are preferred skills but not required.
Work Conditions & Physical Demands: Weekend work is required. Role requires an alternating schedule of Wednesday-Saturday and Tuesday-Friday. Requires flexibility in working in the community (inside and outside) at various locations within Cobb and Douglas Counties. May work outside in hot or cold temperatures. Must have reliable transportation. Administrative functions can be completed remotely from home and/or in various CDPH satellite offices as needed. Some physical exertion is required. May require some lifting (no more than 25 pounds) and may require standing for periods of time. This is a part-time position (up to 24 hours per week).
Salary Information: $20.70/Hr. Up to 29 hrs./ week | No Benefits

- Assists residents with scheduling appointments, seeking health resources, and linking to credible educational resources.
- Conducts outreach and provides education addressing COVID-19 preventive measures, healthy lifestyles to reduce the risk of various chronic diseases, and overall wellness.
