The PM&R Physician will deliver and oversee the rehabilitation care of Valir Rehabilitation Hospital. The PM&R Physician and the physician team work jointly with nursing, therapy, case management and dietitians as part of the Interdisciplinary Team to determine and monitor the care plan for each rehabilitation patient in compliance with all CMS, TJC and state regulations/laws.
Duties/Responsibilities:- Completes required documentation timely.
- Periodic assessments on established participants.
- Evaluation and treatment of participants during acute illness.
- Determining the need and facilitating referrals to other medical specialists when deemed necessary.
- Discussing care guidelines with patients.
- Rotating weekend call with other Valir Rehabilitation Hospital clinical care providers.
- Participates in the development and revision of the plan of care as a member of the interdisciplinary team.
- Communicates participant changes to the team.
- Integrates the primary care treatment plan into the overall plan of care.
- Interacts with other team members to meet emergent and acute needs of participants
- Participates in discharge planning.
- Maintains quality documentation of all services provided.
- Collaborates with nurse practitioner, physician assistant, and/or other physicians to ensure comprehensive and continuous care
- Conducts offsite assessments or attends meetings at our Joint Venture Partners' hospital as needed. (Additional Compensation)
- Is an active participant in Infection Control, QAPI and other committees as deemed necessary by the Hospital Administration.
- Acts as a liaison between Valir Rehabilitation Hospital and community physicians
Education, Licenses, Certifications and Experience:
- Certified to practice medicine in the State of Oklahoma without limitation or restriction.
- 3 years in the post-acute patient care.
- Hospital experience preferred.
