Responsibilities
The Social Worker is responsible for the implementation of standards of care for social work services in the home health and hospice setting.
Essential Key Job Responsibilities
- Assesses the psychosocial status of clients related to the client’s illness and environment and communicates findings to the registered nurse.
- Carries out social evaluations and plans intervention based on evaluation findings.
- Maintains clinical records on all clients referred to social work.
- Provides information and referral services to clients and families/caregivers regarding environmental needs and
- Provides information to clients or families/caregivers and community agencies.
- Markets and promotes the agency programs and events in the local community.
- Serves as liaison between clients or families/caregivers and community agencies.
- Maintains collaborative relationships with organization personnel to support client care.
- Maintains and develops contracts with public and private agencies as resources for client and organization personnel.
- Participates in the development of the total plan of care and case conferences as required.
- Assists medical team in understanding significant social and emotional factors related to health problems.
- Participates in discharge planning.
- Coordinates new volunteer training, continued education, and annual evaluations
- Assigns volunteers to each hospice family and coordinates visits
- Coordinates the bereavement program.
- In conjunction with the agency director, the Social Worker will be the contact person with the foundation department at the local hospital.
- Responsible for event coordination, planning and execution.
- Assures proper documentation of social services in clinical records in compliance with state, federal, and Joint Commission standards.
Qualifications
Graduate of a bachelor’s program in social work accredited by the council on social work education
Licensed as a Social Worker in state of practice
One year experience in healthcare
Preferred
Experience in Health at Home and Hospice
Pay Range
$23.88 - $36.92 /hour
