Community Hospice is a locally-owned and managed hospice agency serving the community since 1996. We are searching for an experienced Registered Nurse to provide compassionate care to our patients in both facilities and home locations. This position is responsible for identifying patient/family needs and for providing supportive care and appropriate services in accordance with the attending physician's order and the IDT plan of care.
Job Duties
- Evaluates on an ongoing basis, the patient/family situation with particular attention to the physical needs of the patient and revises the plan of care accordingly
- Educates the patient/family regarding care and other identified needs related to the illness of the patient
- Counsels and provides emotional support to the patient/family during the dying and grieving process
- Assists with the coping of the patient/family through anticipatory guidance of events throughout the disease process
- Assists in the development of the IDT plan of care by participating in team meetings and patient care conferences
- Maintains close communication with the Clinical Manager
- Maintains close communication with the patient's physician reporting all significant changes in patient's status and collaborates with the physician to develop treatment protocols, which are consistent with Hospice palliative care guidelines
- Documents care provided in an appropriate and timely manner
- Supervises HHAs / LPNs or other paraprofessionals providing care to the patient/family
- Works non-judgmentally with patients and families of many cultural, religious and socioeconomic backgrounds and various lifestyles
- Other duties as assigned
- Graduate of an accredited school of professional nursing who is currently licensed as a Registered Nurse in the state of Alabama; Baccalaureate degree preferred.
- At least 1 year of professional experience in the delivery of nursing services, preferably 2-5 years
- Current CPR certification
- Self-disciplined with ability to multi-task and manage tasks as they arise
- Sensitivity to the dynamics of death, dying and bereavement
- Commitment to the hospice philosophy of care
- Current driver's license and dependable transportation
