This Full Time Hospice RN Case Manager will serve patients from Crestview/Holt to Milton Pace.


Position Summary:

The RN Case manager will identify patient/family needs, provide supportive care, and appropriate services through skilled nursing assessment, planning, and care in accordance with the attending physician’s orders and the interdisciplinary team (IDT) plan of care to maximize the comfort and health of patients and families consistent with Peoples Hospice policies and procedures.


Essential Duties & Responsibilities

  1. Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing, and evaluating phases of the nursing process.
  2. Initiate start of care including collaborating with hospice medical director on patient appropriateness, providing information to patient and family, establishing initial plan of care, and obtaining start of care orders.
  3. Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
  4. Maintain regular communication with the Administrator to review caseload.
  5. Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions.
  6. Obtain data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care.
  7. Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.
  8. Maintain up-to-date patient records so that problems, plans, actions and goals are accurately and clearly stated, and changes are reflected as they occur.
  9. Instruct primary caregivers, volunteers, and employed caregivers to provide care as indicated.
  10. Meet regularly with hospice nursing staff to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed toward enhancing professional growth.
  11. Attend patient conference and facilitate discussion of issues from caseload for full staff discussion, consultation, and evaluation.
  12. Supervise and document observance of the hospice aide delivery of care every 14 days.
  13. Inform the Administrator of unusual or potentially problematic patient/family issues.
  14. Assist other nurses including assuming primary responsibility for patients when necessary.
  15. Share in providing 24-hour, seven-day-a-week coverage to patients/families when necessary.
  16. Provide appropriate support at time of death and perform bereavement assessment if requested.
  17. Participate in PHPC's orientation and in-service training programs for professional staff.
  18. Participate in agency and community programs as requested to promote professional growth and understanding of hospice care.
  19. Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which aid in determining policies.
  20. Reviews and signs incident reports.
  21. Complete discharge notes/forms in the medical record on planned discharges.
  22. Accept other assignments as appropriate.


Requirements:

  1. Current valid FL RN license.
  2. Minimum two years of recent varied work experience as a professional nurse, preferably one of the two years in an acute setting.
  3. Reliable personal transportation.
  4. Valid driver's license and auto insurance.
  5. Demonstrated ability to assess and respond to the needs of patients and families in varied settings.
  6. Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.

Work Environment

While performing the duties of this position, the employee travels by automobile and is exposed to changing weather conditions. The employee works in varied residential environments based on assigned caseload.
