We are seeking a Hospice RN Case Manager to join our team! You will be responsible for the oversight of care and administration of nursing care to a patient caseload of 10-12 terminally ill patients in their place of residence. You will coordinate care with the interdisciplinary team, the patient, the family and the referring agency.
Responsibilities:
- Administer nursing care to terminally ill patients
- Establish patient care plans with the interdisciplinary team
- Monitor and report changes in patient symptoms
- Communicate with collaborating physicians and Hospice Medical Director regarding patient care
- Provide end of life education to all patients and their loved ones
- Maintain accurate patient medical records
- Provide advice and emotional support to patients and their family members
Qualifications:
- Previous experience in nursing or other medical fields
- Familiarity with electronic medical records
- Ability to build rapport with patients
- Strong problem solving and critical thinking skills
- Ability to thrive in a fast-paced environment
