HH Case Manager RN Contract
- 13 weeks with extension options
- 40 Guaranteed hours per week
Hospice Case Manager RN Shift
- Day shifts
- 8 hour shifts (8a-4:30p)
- 1 wknd rotation/month
- No on-call
Experience: 1+ years
Charting System : EPIC (experience required)
- Patient Types:
- Staff will primarily be seeing patients in the community setting, i.e. their home, the home of a family member, Assisted Living Facilities, Group Homes, Nursing Homes
- High acuity patients: pleurx drains, PCA pumps, IV ABX, TPN, Foleys, wounds, Cancer
