- Travels to patient's current location (hospitals, skilled nursing facilities and assisted living facilities) to facilitate initiation to hospice service
- Identifies objections, concerns or other entry barriers and provides information in response to address those issues.
- Describes and explains services to potential patient/family and elicits responses to questions regarding patient's fears, physical limitations and attitude toward hospice care in order to put the patient and family at ease about choosing hospice and to gather pertinent information necessary for admission.
- Speaks with patient/family regarding financial obligations and insurance coverage, using knowledge of the Medicare Benefit as well as other Medicare and other insurance reimbursement.
- Obtains necessary patient consent and pertinent admission forms such as the Medicare Election of Benefit.
- Associates degree required; Bachelor's degree in social work preferred. An equivalent combination of education and experience may be substituted at the discretion of the Administrator.
- Customer service background. Strong sales skills with ability to assist families in choosing the hospice options.
- Satisfactory completion of background screening or other consumer reports, when applicable in compliance with Hospice of the Comforter policy and AHCA regulations.
- Satisfactory compliance of Hospice of the Comforter's drug free workplace policy.
- Participate in appropriate continuing education and training as required by supervisor.
- Successful completion of the Hospice of the Comforter Orientation and Training, as appropriate.
- Maintain a current, valid Florida Driver's License in good standing with a safe driving record and a reliable vehicle.
- Maintain valid personal automobile insurance
The Hospice Program Representative facilitates the registration of patients to the hospice service in the hospital. The Hospice Program Representative is responsible for obtaining signed consents for the Hospice Program through meeting with/providing hospice solutions to prospective patients and families. The Hospice Program Representative will document the encounter and outcomes in the patient's chart utilizing the electronic medical record.
