We are seeking a Licensed Vocational Nurse to join our Home Hospice Team. As an LVN, you will play a vital role in providing quality care to patients in their homes. This is a rewarding opportunity to make a difference in the lives of individuals in need of medical assistance.
```Qualifications:```
•Must have a valid Licensed Vocational/Practical Nursing license within this state
•Must have at least one year of experience as a LVN/LPN
•Must have evidence of CPR training
•Valid driver’s license, auto insurance, reliable transportation, and willingness to drive to patient’s location
•Must be able to lift 50 pounds
•Must clear a criminal background check and physical screening
```Responsibilities:```
•Demonstrated competency in performance of skilled nursing functions
•Ability to work with dying patients and their families
•Under the direction of a Registered Nurse, the Hospice field nurse provides skilled nursing and palliative care for assigned Hospice patients
•He/she works cooperatively with the other members of the Interdisciplinary Hospice Team
•The Hospice nurse provides intermittent visits or shifts of skilled nursing care, performs prescribed medical treatments, including pain management and symptom control, provides nursing observations, provides teaching regarding the expected course of the disease, care techniques and other health measures to the patient, family or other primary caregiver, and participates in the care plan development and implementation
```Experience:```
Experience as a Home Hospice LVN is preferred, but not mandatory
