Concordia Hospice of Washington serves our patients in Washington and Greene counties with the focus to provide services that offer comfort, peace, and dignity for those with incurable illnesses and for their families.

Concordia Hospice of Washington is seeking a Licensed Practical Nurse (LPN) for a full-time position which will cover our Multiple Areas within our service community.
POSITION SUMMARY
The Licensed Practical Nurse (LPN) works under the guidance of the Registered Nurse (RN) as a member of the hospice team to give direct nursing care to hospice patients in accordance with the policies and procedures of Concordia Hospice of Washington. Care for the patient is given where the patient resides (i.e., patient’s home, facility, hospital). The activities of the LPN are based on the employee’s education, preparation and experience.
