Concordia Hospice of Washington is seeking a Licensed Practical Nurse(LPN) for a full-time or part -time position which will care for our Hospice patients in our South Hills service area. A wonderful coverage area to accommodate your work life balance.
POSITION SUMMARY
The Licensed Practical Nurse (LPN) works under the guidance of the Registered Nurse (RN) as a member of the hospice team to give direct nursing care to hospice patients in accordance with the policies and procedures of Concordia Hospice of Washington. Care for the patient is given where the patient resides (i.e., patient’s home, facility, hospital). The activities of the Licensed Practical Nurse (LPN) are based on the employee’s education, preparation and experience.

RESPONSIBILITIES
1. Works with the Interdisciplinary Group (IDG) to develop, coordinate, and update the individualized plan of care for hospice patients/families cooperatively with the patient and/or family, physician and hospice team
2. Coordinates and communicates with the hospice team the weekly patient schedule to meet the needs of the patient and family
3. Completes a supervisory visit on the Hospice Aide as required and oversees the appropriate implementation of the individualized plan of care by the Licensed Practical Nurse (LPN), Hospice Aide and Volunteer
4. Conducts a physical examination and nursing assessment of the hospice patient at each skilled nursing visit
5. Serves as the primary nurse for patient case load coordinating the total patient care, evaluating the effectiveness and the appropriateness of the plan of care, making changes when necessary
6. Provides personal care such as (but not limited to): bathing, grooming, dressing, shaving, feeding; assessing vital signs; assisting with ambulation; changing position in bed; assisting with transfers, changing bed linens; assisting with elimination; providing oral hygiene, skin care, foot care, ear care, light house cleaning and meal preparation
7. Together with the hospice team, provides care as established by the IDG to meet the physical, emotional and spiritual needs of the patient as necessary, and appropriately supports the family at the time of death
8. Completes all computer and/or paper documentation requirements within the timeframes specified
9. Participates in required on-call responsibilities
10. Attends and participates in IDG meetings, staff meetings and other related activities when required
11. Completes required in-services; ensures all required continuing education credits are obtained for licensure renewal
12. Demonstrates an understanding of the Quality Assessment Performance Improvement (QAPI) process, hospice policies and procedures, and participates in projects when requested
13. Participates in the orientation of new staff members when requested
14. Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards, and Concordia Hospice of Washington policies and procedures; maintains confidentiality of patient and employee records and information; has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to the Executive Director and/or Compliance Manager
