GENERAL JOB SUMMARY:

  • This position is responsible for providing end-of-life spiritual supportive services to AMHA Hospice patients. These services must be consistent with the individual patient’s and family’s beliefs, practices, religion, faith, custom, culture and ethnicity. Spiritual care services may include patient visits in the home or facility. The Chaplain may also make contact with clergy in the community. This position does not carry supervisory responsibilities.

ESSENTIAL FUNCTIONS:

  • Ensure that Hospice philosophy of care as defined by state and federal regulations is appropriately implemented.
  • Provide patient assessments at the location that the hospice services are delivered per regulations.
  • Gather the essential information necessary to begin the plan of care and provide for immediate spiritual care and services when requested and as required by regulations.
  • Communicate the patient’s changing spiritual needs and plan of care to team involved in that care, utilizing minimum necessary use of PHI and conditions of access.
  • Document the patient’s spiritual needs and complete progress notes as required by hospice guidelines in a timely manner.
  • Provide emotional and spiritual counseling to patients and their families.
  • Provide the patient or family with verbal or written resources that may be helpful to their spiritual experience as appropriate.
  • Encourage families to make arrangements in advance regarding wills, funeral arrangements, obituaries and internment etc.
  • Provide direct spiritual counseling, prayer, liturgical and sacraments to patients and their families when requested.
  • Make an effort to be present at the patient’s time of death when possible and when requested.
  • Organize and conduct special ceremonies such as memorial services, funerals, or life celebrations.
  • Collaborate and communicate effectively as part of the team to establish and maintain cooperative working relationships.
  • Participate as a member and attend IDT meetings and document as required.
  • Be a spiritual resource to team members assisting them with their grief experience.
  • Participate in and complete all continuing education as assigned in a timely manner.
  • Engage with other clergy and community groups to establish and maintain cooperative working relationships.
  • Provide counseling in a respectful and dignified manner.
  • Practice safety in the workplace.
  • Maintain patient confidentiality at all times.
  • Advocate for patient’s best interest.
  • Fluent in the English language. Bilingual a plus
  • Ability to position self to move about, ascend, descend or traverse, transport, operate, activate, use, prepare, inspect or detect, perceive, identify, recognize, observe, inspect and assess are required. Ability to convey, communicate, speak clearly and exchange information, converse with, discern, and express oneself.

JOB REQUIREMENTS AND QUALIFICATIONS:

Education::

  • Bachelor's degree in Divinity, Theology, Ministry or Pastoral Care from an accredited school.
  • Minimum of two years experience providing spiritual support and counseling to clients and family members in a health care setting.
  • Demonstration of expertise in spiritual assessment and counseling, and in concepts of death and dying issues.

Experience::

  • Preferred previous hospice experience.

Knowledge Requirements:

  • Hospice Philosophy of care; working knowledge of current hospice guidelines.
  • Standard concepts and practices performed by Chaplains.
  • Human behavior and performance, individual differences in ability, personality and interests.
  • Different philosophical systems and world religions including their basic principles, values, ethics, ways of thinking, customs and practices.
  • General knowledge of crisis intervention techniques and community disaster response.
  • Knowledge and support of agency’s safety and OSHA policies.
  • Understanding of HIPAA Privacy Standards and related agency policies.
  • Demonstrated understanding and compliance with agency policies, including but not limited to documentation, continuing education, work schedules and required HR documents.
  • Signs and symptoms of suicide ideation.
  • Signs and symptoms of substance, drug and alcohol abuse.
  • Patient’s Rights.
  • Advance Directives.
  • Customer Service principles.

Skills and Ability::

  • Moderate computer skills in Microsoft Office applications.
  • Structure and content of English language including the meaning and spelling of words, alphabetization, rules of composition and grammar.
  • Calculate general math computations.
  • Analyze information and evaluate results to choose the best solution.
  • Utilize proper body mechanics and safe working techniques.

Job Type: Part-time

Pay: $52,000.00 - $60,000.00 per year

Schedule:

  • 8 hour shift
  • Monday to Friday
