Hospice Chaplain will be responsible for providing spiritual care to patients and families of all faiths. This Chaplain would provide prayer, reading, active listening, funeral arrangement planning and other tasks as stated in the following job description:
Environmental and working conditions: Works in patient homes and other healthcare facilities under various conditions. Occasional exposure to harsh weather conditions. PPE will be provided but exposure to infectious diseases could occur.
Essential Functions:
*One year of spiritual counseling in the healthcare field preferred.
*Assess hospice patient and support system to identify spiritual needs as evidenced by the plan of care, documentation, clinical records, and IDG conference minutes.
*Provide support for the spiritual needs of the patient/family.
*Attend IDT meetings and participate in the patient care planning processes.
*Demonstrate commitment, professional growth and competency by communicating and being involved within the agency as needed.
*Demonstrate understanding of coordination and continuity of care by collaborating with the IDT team.
*Meet mandatory continuing education requirements of the agency.
