Under the supervision of the Regional Director or designated RN, the Hospice Aide performs basic patient care activities necessary in caring for the personal needs and comfort of terminally ill patients in various settings. Works along with the interdisciplinary group (IDG) to provide quality patient care, weekend and holiday work is required.
All employees assigned this job title must be willing and able to flex their work location within and between any Community & Palliative Care) locations.
Primary Responsibilities:
- Performs personal care activities to promote and maintain patient hygiene.
- Assists patients in performing necessary activities of daily living.
- Assists patients with ambulation and exercise.
- Uses proper body mechanics to promote patient safety.
- Maintains a safe patient environment to prevent patient injury
- Provides care in an emotionally supportive approach.
- Keeps Interdisciplinary Group (IDG) members abreast of patient and caregiver status, changes, and or concerns.
- Provides nutritional support to patients. Assists in post-mortem care.
- Participates in mentoring new employees.
- Participates as an active member of IDG and participates in meetings, when appropriate.
- Attends and participates in regularly scheduled staff meetings.
- Works with all Community Hospice departments to ensure compliance with Community Hospice objectives, plans and procedures.
- Maintains a neat and clean environment while carrying out all duties.
- Provides light housekeeping, and meal preparation as directed in the patient plan of care.
- Demonstrates knowledge of standard precautions and ability to perform basic infection control procedures.
- Demonstrates to family/caregiver proper techniques to meet the personal care needs of the patient.
- Performs personal care activities to promote and maintain patient hygiene.
- Reports changes in patient's condition or care needs that require follow-up to supervisory and/or appropriate team member. Can include physical, spiritual, social and emotional status.
- Documents care provided in accordance with organization standards
- Maintains patient confidentiality at all times.
- Attends team meetings and participate in other organization meetings as directed.
- Maintains a minimum of twelve (12) hours of training/education specific to position annually.
- Other duties as assigned.
Required Licenses/Certifications:
- Current licensure as a Certified Nursing Assistant (CNA) in the state of Florida.
- Current State of Florida Driver's license with driving record acceptable to Community Hospice insurance carrier; reliable transportation.
- Current AIDS 104 Certification.
Qualifications:
Education/Experience:
Any combination of education and experience that would provide required skills and knowledge for successful performance would be qualifying. Typical qualifications would be equivalent to:
- High school diploma or GED equivalent preferred.
- Must have six (6) months experience in home care, hospital, and/or long term care.
- Experience in caring for the terminally ill preferred.
- Reliable transportation that allows employees to shift work locations when needed.
- Furnish one or more active telephone numbers that enables management to contact employee should an unexpected change of work location be required.
