Summary
We are seeking a compassionate and dedicated individual to join our hospice team as a Hospice Aide. As a Hospice Aide, you will play a crucial role in providing personalized care and support to patients in the comfort of their own homes. Your primary responsibility will be to assist patients with their daily living activities, ensuring their comfort, dignity, and well-being. This role requires excellent communication skills, a deep sense of empathy, and the ability to work collaboratively with a multidisciplinary team.
Responsibilities
· Provide direct personal care to patients under the supervision of a registered nurse or other healthcare professionals
· Assist patients with activities of daily living (ADLs) such as bathing, grooming, dressing, and toileting.
· Help patients with mobility and transferring, ensuring their safety and preventing falls or injuries.
· Support patients with feeding and provide assistance with proper nutrition and hydration.
· Administer medications as directed by healthcare professionals and document accurately
· Monitor and report any changes in patients' condition or behavior to the supervising nurse.
· Assist in maintaining a clean and safe environment for patients, including light housekeeping duties.
· Document patient care activities, observations, and interventions accurately and in a timely manner.
Qualifications
· High school diploma or equivalent.
· Completion of a state-approved Hospice Aide or nursing assistant training program.
· Current certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA) preferred.
· Minimum of 6 months of experience in a healthcare setting, preferably in hospice care or a related field.
