Bridges Hospice is GROWING and hiring an additional Admission RN to join our amazing team in the Westmoreland/Allegheny County areas! The position will be 4 days a week position. 10-hr days which will include Saturdays.

As an Admissions RN, you will assess patients for hospice eligibility and explain the hospice benefit to patients and families. Manage any immediate symptom and provide education to the patient and/or the patient.
POSITION SUMMARY:
- Support patients with change or decline.
- Perform visits to patients on the Bridge Program.
- Provides education on medication changes, symptom management, emotional support to patients/families and caregivers.
- Responsible for assessing patients' eligibility for hospice and admits patients as appropriate.
- Initiate the individualized initial plan of care in collaboration with the attending physician and hospice medical director.
- Provides skilled nursing care focusing on pain control and symptom management.
- Educate patients, family members and facilities about the Medicare Hospice benefit.
- Ensure all patients and families initial needs are met.
- Collaborate and communicate with the Interdisciplinary Team members regarding initial plan of care, patient’s condition, and family support needs.
- Contact and maintain relationships with long-term care staff, hospital staff, and physicians to promote hospice services.
- Provides immediate guidance to the patient and his or her family regarding diagnosis, medications, treatments and the patient's progress.
POSITION QUALIFICATIONS:
- Graduate of an accredited school of nursing.
- One (1) to two (2) years of recent Hospice care experience is a must.
- Current registered nurse (RN) license, and driver's license.
- Must be willing to operate personal vehicle as necessitated by the position.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills.
Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients.
Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. Visual acuity and hearing to perform required nursing skills.

Pay: $68,640.00 - $76,960.00 per year
