The Pharmacist will work onsite in Houston, TX. Call rotation is every 6 weeks for 1 week at a time (Friday - Thursday).


Primary Responsibilities:

  • Review medication orders and evaluates appropriateness and completeness of orders with consideration given to the special needs of Home Infusion customers
  • Verifies all prescriptions not directly received from the prescriber or physician’s agent
  • When required, compounds intravenous and specialty pharmaceuticals in an aseptic fashion according to established State Board of Pharmacy, Federal, USP 797/800 sterile guidelines and company policy Knowledgeable of the proper use and handling of both sterile and hazardous medications
  • Compounds and handles sterile and hazardous medications in accordance with established guidelines and regulations
  • Provide oversight of the safety; appropriateness, and effectiveness of the pharmaceutical care provide to patients (consumers) as it relates to specialty products dispensed
  • Assess the patient at the initiation of drug therapy, develops a patient care plan and drug monitoring regimen and monitors the patient according to the care plan throughout the course of therapy Communicate with the patient, physician, nursing staff, other pharmacists, and other members of the health care team regarding patient status, response to therapy and clinical objectives to ensure clinical outcomes are met
  • Ensure compliance with all FDA, DEA, Texas State Board of Pharmacy, OSHA and other regulatory standards Follow all department policies and procedures Provide oversight of pharmacy technician and pharmacy support staff

Required Qualifications:

  • BS in pharmacy or PharmD degree
  • Pharmacist license in good standing in the state of Texas
  • Sterile Compounding Certification obtained through completion of a single course, a minimum of 20 hours of instruction
  • 1+ years of experience in Home Infusion therapies, hospital pharmacy experience (working with compounded sterile products in compliance with USP 797/800) OR Completion of ACPE accredited PGY1 residency
  • Demonstrates solid understanding of clinical and dispensing practices for intravenous pharmaceuticals
  • Computer and typing proficiency, including MS Word, MS Excel, and other pharmacy-dispensing and documentation related software programs
  • Proven ability to participate in the on-call rotation and, after-hours prescription dispensing
  • Ability to work on-site daily on the Houston, TX location


Preferred Qualifications:

  • BCNSP or BCSCP certified
  • Pharmacist license in good standing in other states, besides Texas
  • 3+ years of sterile compounding experience in compliance with USP 797/800 guidelines
  • 3+ years of Home Infusion therapy experience
  • 3+ years of experience processing and managing TPN, Inotropes, and Antibiotics
  • Experience with legal and regulatory requirements/visits for: TSBP, URAC, ACHC, CMS, NAPB, etc.
  • CPR+ software experience (not BLS or ACLS)
