- Assists with ambulation, transfers, and repositioning in a safe manner.
- Observes basic infection control procedures.
- Obtains vital signs as indicated on tech care plan and reports abnormal results to primary clinician or Case Manager.
- Provides personal care such as bathing, shampooing, mouth care, skin care, linen change, and foley care.
- Reports and documents any changes in patient condition to the appropriate clinician or Case Manager.
- Certified Nursing Assistant or Registered Home Health Aide required.
- Demonstrated oral and written communication skills.
- Ability to manage time, work independently, establish priorities and to communicate with a team.
- Ability to provide professional, courteous, personable, and a positive attitude.

EDUCATION
- High School Diploma/GED Equivalent Required
- 1 Preferred
- Certified Nursing Assistant
- Home Health Certification
- Certified Cardiac Pulmonary Resuscitation
