General Description:
Provides professional comprehensive nursing service to assigned patients in accordance with the objectives, policies, and procedures of this agency. Has the responsibility, accountability, and authority for providing coordination of services of all other disciplines and services.
Essential Job Duties:
1) Responsibilities/Performance Criteria:
a) Professional responsibilities of case management.
i) Acts as primary nurse for assigned patients.
ii) Coordinates care with other disciplines and other agencies.
iii) Communicates care with physicians, team members, nurse manager, patient and family.
b) Clinical Abilities
i) Demonstrates the ability to provide a nursing assessment of the patient’s health status, which includes the physical, psychological, and social/family aspects.
ii) Develops the plan of care to reflect achievable, measurable, expected outcomes.
iii) Maintains accurate and complete medical records and updates information in a timely manner. Follow agency policies/procedures and standards and keeps abreast of technical/procedural skills through attendance at in-service training and workshops.
iv) Demonstrates and teaches needs to patient/family and caregivers.
v) Develops plan of care for Home Health aide, instructs and supervises them regarding individual patient needs.
2) Personal
a) Displays organization in approach to time and tasks.
b) Delegates appropriate tasks to support personnel.
c) Able to prioritize tasks.
d) Able to make independent decisions.
e) Able to analyze a situation and develop a workable plan.
f) Displays flexibility and can adjust schedule if an unexpected problem occur
3) Knowledge of Communication Skills and Functions within HHCS System
a) Understands/complies with rules and regulations governing this agency.
b) Keeps nurse manager informed of major changes in patient’s condition.
c) Submits physician orders and medical updates in a timely manner. Keeps working chart information updated.
d) Completes written documentation effectively reflecting skilled care provided. Provides notes for filing within 5 working days.
e) Shares new concepts and ideas with peers and staff. Participates in meetings and in-services.
f) Displays good interpersonal communication skills with: patients/families; colleagues; physicians.
g) Assumes responsibility for agency call and scheduled weekend visits as assigned.
h) Displays an ability to present a positive image for the agency in the performance of day-to-day activities.
i) Attends all mandatory in-services and keeps CPR certification current.
j) Participates in Performance Improvement program by continually evaluating patient care and utilizes feedback from said studies in improving practice.
4) Performs other related duties as assigned
Work/Education Requirements:
Graduate from an accredited School of Professional Nursing.
Eligible for or currently licensed as a registered professional nurse in Pennsylvania.
Minimum of 2-4 recent years of Medical/Surgical experience, or previous Home Care experience
Current drivers license and a car available.
Displays evidence of independent nursing practice in delivering skilled nursing care required. Demonstrates the ability or be willing to learn to perform highly technical skills. This includes operating infusion pumps, accessing central venous devices, or using monitoring apparatus.
Demonstrates the ability to deal effectively with stressors in a positive manner. This includes dealing with disagreeable situations in a nonjudgmental manner.
Demonstrates the ability to perform physical tasks that require heavy lifting, extensive bending and standing.
Job Type: Full-time
Pay: $29.22 - $36.44 per hour
