Home Health: Physical Therapist (Full Time) - Punta Gorda

Responsibilities:

- Conduct comprehensive evaluations of patients' physical abilities and limitations

- Develop and implement individualized treatment plans to improve mobility, strength, balance, and overall functional independence

- Provide skilled therapeutic interventions, such as therapeutic exercises, manual therapy techniques, and assistive device training

- Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies

- Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic care

- Document and maintain accurate records of patients' progress and treatment plans

Qualifications:

- Active and unrestricted Physical Therapist license in the state of Florida

- Minimum of 1 year of experience in physical therapy, preferably in a Home Health or a related setting

- Strong clinical skills and ability to provide patient-centered care

- Excellent communication and interpersonal skills

- Ability to work independently and make sound clinical judgments

- Proficiency in electronic medical record (EMR) systems

- Valid driver's license and reliable transportation to travel to patients' homes.
