Essential Duties and Responsibilities:

As an In-Office Licensed Vocational Nurse, you will ensure quality care is being provided to patients by reviewing clinical notes to make sure quality assurance standards are met.

• Transcribe/attach all orders from Forcura, plot visits from orders, including coordinating with field staff on changes, including field staff requested orders
• MD orders and calls (including verbal orders for SOC)
• Reviews all lab, x-ray results/communicates (fax unless critical) to MD & field staff
• Review of F2Fs prior to admissions.
• Maintain inventory/ordering supplies
• Fax/Request all medical records/F2F needed from hospitals.
• Help with day-to-day scheduling (SOCs)
• Management and tracking of past due notes.
• Upload Consents, EPP, Release of INFO, Medication Profile, NOMNC into EMR system
• Assists with Case Conference management
• Assures the confidentiality of clinical records.
• Participates in the development and implementation of a staff educational program for documentation and Quality Improvement.
• Works with administrative and supervisory personnel regarding home care visits to assure high quality and proper follow-up patient care regarding patient needs.

Qualifications

• Have at least 1 year of Home Health experience within the last 24 months and have a demonstrated understanding of quality assurance requirements related to home care services
• Communicates clearly and effectively (both oral and written) with patients, clients, Team Members, peers, and Leadership.
• Excellent documentation and organizational skills, including time management, multi-tasking, and prioritizing
• Ability to establish and maintain effective working relationships with all segments of the company
• Exhibit self-direction, independent judgment, discretion, and ability to work with multiple teams
• Must maintain a professional and friendly demeanor
• Kinnser/Wellsky Experience Preferred
