Granite VNA is hiring Home Health LNAs! This is a Per Diem job opportunity. Hours are typically from 9 a.m. - 1 p.m. Monday - Friday. Hours and days per week are flexible.

The Home Health LNA helps achieve our mission by performing personal care and designated health-related services for clients in their homes under the supervision of a Registered Nurse. The Home Health LNA interacts with client's family members, facilities and other healthcare team members while maintaining professional standards. Some essential job functions include:
- Follows care plan in providing basic care and services to the client including assistance with Activities of Daily Living (ADL): walking, bathing, shaving, mouth care, grooming, eating transfers, and management of incontinence.
- Communicates with nurses involved in the resident's care plan; observes, records and reports changes in resident's condition, status, or other pertinent information to the nursing staff clearly, concisely and in a timely manner.
- Documents care provided to the resident accurately in accordance to the agency's guidelines.
- Complies with the Agency visit documentation, time entry and electronic scheduling requirements.
- Assists client with homemaking tasks, medication assistance, and wound care according to the patient's care plan.
Requirements
- High school diploma/GED, accompanied by current New Hampshire LNA license
- One year of recent experience as an Licensed Nursing Assistant
- Reliable automobile and auto insurance
- Valid Driver's license
