You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, providing personalized and attentive care and building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.
- Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
- Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
- Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
- Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
- Give medications and immunizations
- Engage patients in exercises or other activities
- Graduated from an accredited Home Health Aide program
- High School Diploma or GED
- One year prior professional experience
- Driver’s license required
- CPR certification required
RESPONSIBILITIES:
- Must HaveHome Health Intake Office Experience
- Receive/acknowledge incoming referrals and medical orders
- Build strong relationships with referral sources during the intake process
- Daily CRM & EMR data entry
- Collaborating closely with the scheduling team to staff new admits
- Insurance Verification and Authorization
- Medicare, Managed Care, Medicaid, and VA Verification experience required.
- Scheduling initial nursing and therapy visits, as per Medicare guidelines
- Follow-up with patients before, during, and after admission
- Prepare patient charts, HR files, and office documents
- Contact via telephone/fax/email with referral sources, doctor offices and facilities
EXPERIENCE:
- Data Entry: 2 years (Required)
- EMR Systems: 2 years (Required)
- Insurance Verification: 3 years (Required)
- Home Health Intake: 2 years (Required)
- Medical Office: 2 years (Preferred)
JOB TYPE:

SALARY:
- $20.00 - $25.00 per hour
