SalaryWe are seeking a Home Health IHCC Care Coordinator for our office in Bedford, IN! The purpose of the Care Coordinator is to help maintain stabilization, delay/prevent deterioration of health status, manage chronic conditions, and/or improve health status for patients receiving this service.
Job Requirements and Responsibilities:
- Performs medical coordination to manage the healthcare of the individuals receiving this service including physician consults, medication and supply ordering, and helping patients understand new or existing diagnoses.
- Development and oversight of a healthcare support plan.
- Required to coordinate and collaborate with other care managers and healthcare professionals to ensure quality care management is being delivered.
- Deliver care coordination through weekly consultations or reviews
- Face-to-Face visits with the patient at a minimum of one visit per month
Qualifications/Skills:
- Licensed Practical Nurse (LPN), required
- 1+ year experience in a healthcare setting
- Medicaid Skilled Nursing experience, recommended
- Proficient knowledge of Microsoft Office and EMR systems
- Valid Driver's License
- Must pass a background check
