Form of workWe are looking for a Full-time Nurse Practitioner (NP) for a very flexible position as a Hospice and Palliative Care Nurse Practitioner in the home setting. Both experienced Nurse Practitioners and new grad Nurse Practitioners are welcome. We are looking for a NP with a personality that will match the serious illness care setting. The Hospice and Palliative Care Nurse Practitioner works in collaboration with the Medical Director and Hospice/Palliative Team to provide Hospice and Palliative care services to patients and families. The Nurse Practitioner will also perform procedures for which he or she is privileged to do and will document assessments, evaluations, consultation reports and progress notes. Successful outcomes of this position include high quality patient care provided in a compassionate, competent, and caring manner; clinical program development; skill development for Hospice and Palliative clinical staff, regulatory compliance with proper documentation enhanced community relationships, program visibility; excellent staff and patient satisfaction. This Hospice and Palliative Care Nurse Practitioner position is with a Hospice and Palliative company that has been in business many years, so they offer stability and job security.

REQUIREMENTS FOR THE HOME HOSPICE AND PALLIATIVE CARE NURSE PRACTITIONER POSITION: Both experienced Nurse Practitioners and new grad Nurse Practitioners are welcome. The Nurse Practitioner must be licensed in the state. The Nurse Practitioner must be willing to do full-time hours.
