Alamosa County Public Health is seeking a Certified Nurse Aide for our Home Health Agency. Position provides personalized skilled care to SLV Home Health patients in their home. Current CNA certification required.

FLSA Status: Non-Exempt
Salary Range: $23.30 per visit/ $18.35 per hour, non-visit activity
JOB SUMMARY:
Position provides personalized skilled care to San Luis Valley Home Health patients in their home.
ESSENTIAL JOB FUNCTIONS: (The following are illustrative of essential functions to perform duties and responsibilities of the job. - Performs personal care/activities as assigned on a written care plan under the supervision of a Licensed Practical Nurse (LPN) or Registered Nurse (RN).
- Provides personalized care while encouraging independence and self-care.
- Assists patients with activities of daily living, including, dressing, bathing, grooming, toileting and medication reminders.
- Provides mobility and transfer assistance, which may include using assistive devices.
- Promptly reports changes in patient condition to the Case Manager. Case manager will discuss with QA/QI RN if needed.
- Communicates effectively with all members of the patient team.
- Available for on-site supervisory visits with the LPN, RN or Physical Therapist (PT).
- Attends 80% of scheduled staff meetings.
- Completes and provides required documentation of 12 hours of CEU annually.
- Participates in QA/QI activities as requested.
- Records visit and pertinent information in the patient record and synchs the information to the office daily.
- Respects patient rights and is respectful of patient and family needs at all times.
- Maintains confidentiality at all times.
- Maintains current licenses and certifications.
- Performs other duties as required.
REQUIRED QUALIFICATIONS:
- Valid Colorado Driver's License required.
- Must be at least 18 years of age.
- Ability to effectively communicate verbally and in writing.
- Is knowledgeable of patient rights.
- Working knowledge of MS Office and other software, able to apply knowledge of computers within secure databases, able to learn new technologies.
- Ability to create trusting, therapeutic relationships while maintaining professional boundaries.
- Bilingual, English/Spanish preferred.
EDUCATION AND EXPERIENCE:
- One-year experience in Home Health preferred.
- NIMS certification required; can be obtained at job. Must complete within 60 days of hire.
- Current Colorado CNA certification
- Current CPR

Position requires ability to stoop, bend, kneel, crouch, carry, twist and push and/or pull light to moderate amounts of weight. Employee must be able to reach and manipulate objects, tools or controls and to drive. The employee is required to communicate both orally and in writing. Position requires the ability to operate a computer keyboard and standard office equipment at efficient speed. The noise level may be very quiet to noisy depending on the situation.
