Fairview Range is looking for a Home Health Aide (HHA) to join our Home Care and Hospice team.

Schedule: You will work 40-71 hours per two weeks, 4 to 6 hour Day shifts. No weekends.

Responsibilities/Job Description:As a HHA, you will be responsible for:
- Assists with the delivery of age specific direct care to patients in the home setting under the supervision of the Home Care Nurse.
- Provides personal care and performs other delegated duties as assigned by the RN or therapist such as skin care, assistance with a home exercise program and assistance with self-administered medication.
- Light housekeeping related to personal cares.
- Documents all services performed.
- Communicates abnormal patient findings, change in condition or incidents to the RN Case Manager or Supervisor.
Required Qualifications
- You are at least 18 years old.
- Successful completion of a nursing assistant training program, Home Health Aide course or its equivalent and/or completion of competency test.
- Current MN driver’s license and insurance.
