The Certified Home Health Aide, under the direction of the Home Health RN, assists with all aspects of personal cares in the home, including but not limited to, assistance with activities of daily living, monitoring of routine vital signs, errands, and routine procedures which may be performed independently by the aide. While performing routine tasks and procedures, the Home Health Aide gathers data and reports significant information about the patient to the RN and confers with the RN when necessary for direction and instructions. The Home Health Aide documents as required by department, maintains established departmental policies and procedures, safety, environmental and infection control standards and gives courteous care with respect to patient rights and privacy.
Services are provided in the patient’s home. Must provide own transportation. Must provide evidence of automobile liability insurance limits of at least $100,000 combined single limits, or $100,000 / $300,000 / $50,000 split limits.
May be expected to work varying hours, including evening, weekend or holidays to help RN on-call. Performs other duties as requested or assigned.
QUALIFICATIONS
EDUCATION:
- High School Diploma or General Equivalency Degree (GED) preferred.
LICENSE / CERTIFICATION / REGISTRATION:
- Certified Home Health Aide or Certified Nursing Assistant with the ability to obtain Home Health Certification within 6 months.
- Current Valid Driver’s Licensewith no restrictions limiting driving.
- BLS - Obtains RQI BLS certification within appropriate quarter (approximately within 3 months of hire), maintains quarterly RQI requirements.
EXPERIENCE:
- CNA experience preferred, but not required.
