Central Vermont Home Health & Hospice is a full service, not-for-profit Visiting Nurse Association committed to providing high quality, medically necessary home health and hospice care to all central Vermonters, regardless of a person's ability to pay. In addition, we promote the general welfare of all central Vermonters with health promotion, long term care, and maternal-and-child health services.

- You are looking to enjoy a long weekend, every weekend, you'll have Thursday-Sunday's off - The schedule for this role is Monday -Thursday 10am-6pm working hours, plus on call with triage from 6pm-7am. Additional $50 nightly stipend for on call hours, plus hourly rate for in person visits after 6pm.

Your Day-to-Day
- Utilizing your nursing skills to provide hospital level, acute care to patients in their homes.
- Setting care plans goals and calling on the full range of clinical and communication skills to help your clients reach their goals.
- Meeting established nursing visit performance standards to include, but not limited to, OASIS accuracy, productivity, case mix, laptop transfers, etc.
- Participating in regular team meetings as well as monthly and ad hoc meetings with supervisor to discuss case load, challenges, and recommended solutions to identified challenges.
- Collaborating with your colleagues, local physicians, and community providers to provide exceptional health care to our clients.

Requirements
- VT Licensed RN (BSN preferred).
- Minimum of two years' clinical acute care nursing experience required. Home care experience and/or previous case management experience in a Home Care setting and discharge planning and/or utilization review experience preferred.
- Valid driver's license.
Position Highlights
$30,000 sign on bonus for new employees who haven't worked at CVHHH within the past 12 months and who haven't previously received a sign-on bonus, paid quarterly over two years.
