JOB DESCRIPTION
HOME CARE SUPERVISOR
The Center for Elders' Independence is a PACE (Program of All-Inclusive Care for the elderly) organization that uses an interdisciplinary team approach to care planning and care implementation for the purpose of providing high quality, affordable, integrated health care services to the elderly, including an Adult Day Health Center, and promoting autonomy, quality of life and the ability of individuals to live in their communities.
The salary range for the HomeCare Supervisor position at Center For Elders Independence is $ 103K - $ 154K. Wages/salary are based on the market for the HomeCare Supervisor position, as well as experience, skills, abilities and work history
The Position: Under the direction and supervision of the Director of In-Home Services, the Home Care Supervisor supervises the Home Care geriatric aides and Home Care schedulers; assists with the development and implementation of the Home Care geriatric aide and scheduler training programs; supports recruitment efforts for geriatric aides and schedulers; and ensures the adherence to all applicable regulatory requirements. The Home Care Supervisor works in close collaboration with the Home Care Coordinator Nurses to ensure that the care provided in the home meets the needs identified by the Interdisciplinary team and the highest level of customer service is achieved.
DUTIES AND RESPONSIBILITIES:
  • In collaboration with the agency's training coordinator and Home Care Clinical Supervisor, develops/supports and implements a comprehensive training program for the Home Care geriatric aides and Home Care schedulers in accordance with state and federal requirements as applicable.
  • Conducts training as needed for Home Care geriatric aides on site or in participant homes to ensure high quality care delivery and staff education needs are met.
  • Responsible for ensuring that Home Care Geriatric Aide (HCGA) and Home Care Schedulers' timesheets are accurate and completed according to CEI's payroll schedule.
  • Responsible for reviewing and approving any overtime before it is incurred
  • Responsible for completing annual competencies for HCGAs and submitting electronically.
  • Responsible for conducting in home supervisory visits of HCGAs at minimum every 60 days and more often as needed based on employee performance and participant needs, utilizing information and guidance in collaboration with the Home Care nurses or IDT members.
  • Responsible for creating and maintaining an accurate personal care and chore delivery service plan in accordance with the plan specified by the IDT; ensures prompt communication of services to participants and the IDT team; and ensures services are scheduled promptly by the Home Care Scheduler.
  • Ensures that the personal care and chore delivery service plan meets participants' needs through collaboration with providers, HCGAs, Home Care coordinator nurses and Home Care administration team. Providing hands on training, observation, and feedback to Home Care staff whenever necessary.
  • Performs annual performance evaluations for HCGAs and schedulers, at least quarterly one-on-one meetings, or more often as indicated by performance and departmental needs.
  • Ensures compliance with agency policies and procedures.
  • Participates in the revision and review of the Home Care department and relevant agency policies and procedures, job descriptions, orientation processes including relevant training materials and any other written materials as required.
  • Responsible for oversight and delivery of monthly HCGA meetings, including coordination with training/speakers from other disciplines, with input from the Home Care Administration team and Director of In-Home Services.
  • Professionally manages complaints and requests for changes in the service plan.
  • Facilitates and attends Home Care department staff meetings and participates in any other meetings or task forces as assigned.
  • Responsible for HCGA and Home Care Scheduler interviewing and hiring in conjunction with Human Resources partner and Director of In-Home Services. Facilitates Home Care staff orientation - reviews HCGA and Home Care scheduler training manual and completes training for payroll system.
  • Attends job fairs for recruitment of Geriatric Aides as requested.
  • Ensures mileage and payroll is accurate and complete for employees for payroll submission.
  • Attends care conferences for participants and their families.
  • Maintains the confidentiality of all company procedures, results and information about participants, clients, or families.
  • Ensures all services rendered or planned comply with any identified safety precautions necessary to the welfare or interest of CEI, its employees or participants.
  • Works in collaboration with other Home Care Supervisors on projects and assignments as required.
  • Ensures timely communication and collaboration with Director of In-Home Services anytime a concern, complaint or safety matter arises for the Home Care employees or participants and communicates as guided to other departments within the organization.
  • Follows all CEI Policies and Procedures and OSHA (Occupational Safety and Health Administration) safety guidelines.
  • Participates in the agency's quality assurance performance improvement program (QAPI) and any audit or compliance related requirements as assigned.
  • Complies with all agency training requirements.
  • Maintains a courteous, helpful, and professional attitude on the job. Displays a willingness and ability to be responsive in a warm and caring way to all customer groups.
  • Performs other duties as assigned in a positive and helpful manner.

QUALIFICATIONS:
  • Licensed LVN in the State of California preferred.
  • Licensed CNA (Certified Nursing Assistant) in the state of California required.
  • Three (3) years of experience in a supervisory, management or coordinator position in a home health agency or other health related field preferred.
  • Maintains eligibility for licensure obtaining and maintaining required Continuing Education units.
  • A valid California driver's license, current vehicle insurance, and access to a reliable vehicle is required.
  • Demonstrated knowledge and experience working with the frail elderly required.
  • Experience and knowledge of Home Care services including personal care and chore services.
  • Experienced with computer systems, spreadsheets and data compilation.
  • Flexibility, initiative, and ability to work with an interdisciplinary team.
  • Able to handle high levels of stress satisfactorily and be congenial with other employees, customers and vendors at all times.
  • Able to work independently with minimal supervision.
  • Current Basic Life Support CPR certification and First Aid Training required.
