Home Care Physical Therapist

Summary:

The Physical Therapist at Shining Star provides physical therapy assessment, treatment, and education to patients in their homes in accordance with agency policies and medical orders. Both per-diem and full-time positions are currently available.

Responsibilities:

- Evaluate and assess patients' physical conditions and develop appropriate short- and long-term rehabilitation plans.
- Provide hands-on therapy to patients using various techniques and modalities.
- Educate patients and their families on exercises and activities to improve mobility, strength, and function.
- Monitor patients' progress and adjust treatment plans as necessary.
- Collaborate with other healthcare professionals to coordinate patient care.
- Maintain accurate and up-to-date patient records.

Qualifications:

- Doctorate of Physical Therapy from an accredited program.
- Licensed and currently registered to practice physical therapy in New York.
- Minimum of one year's supervised work experience as a Physical Therapist.
- Russian-speaking preferred.

Skills:

- Patient care: Provide comprehensive and compassionate physical therapy services to clients in their home. Demonstrate knowledge of human anatomy, physiology, and medical terminology. Complete and return reports in a timely manner. Develop and maintain relationships with clients and their caregivers and provide regular opportunities for feedback. Ability to move, lift, and transfer patients.
- Interpersonal competencies: Maintain a respectful and communicative relationship with other employees and administration. Ability to assist other employees as needed.

Job Types: Full-time, Part-time, Per diem

Pay: From $100.00 per hour

Expected hours: 20 – 40 per week
