Position Summary:
The Home Care Coordinator works under the general supervision of the Clinical Director and is responsible for organizing the plan of care for participants while providing education and eliminating any potential barriers to treatment. The Home Care Coordinator monitors the quality of care and services being provided to program participants in the home. Ensures continuity of Home Care service delivery at the time of enrollment and throughout participants members as well as compliance with all governing regulations and agency policies and procedures.
Minimum Requirements:
Experience in case management or patient Care Coordination
Two years experience with the frail elderly/geriatric and disabled adults preferred
One-year community based or home health experience preferred
Education/Licenses/Certifications:
Licensed in the State of Washington as an OTR/L
Current CPR
Current Washington State Drivers License: and proof of a safe driving record (which meets established MHS Standards) obtained by a motor vehicle report from the appropriate stateEducation: Per Position DescriptionEmployment Type: Full-Time Regular
