HIV Prevention Case Manager
The Champaign-Urbana Public Health District (CUPHD) is seeking a Case Managerfor a full-time (40-hour week) position in the Champaign office to provide HIV prevention and case management services.
Responsibilities include:
- HIV testing (including phlebotomy), PrEP interventions, counseling in-house and at outreach sites, including the CUPHD mobile program, bar outreach, jail, methadone clinic and other outreach sites as determined.
- Coordinate, evaluate, and maintain supportive and referral services
- Identify resources and referrals for intervention, prevention care management, health and social services for clients
- Spanish speaking are highly encouraged to apply
Additional Responsibilities include:
- Evening and weekend hours will be expected.
- Other assignments will be given as needed to support the teen and adult services team.
Requirements:
- Two (2) years of professional work-related experience.
- Or graduation from an accredited college or university with a bachelor's degree in human services, or a closely related field, is required.
- Persons with lived experience will be prioritized.
- Valid driver's license, dependable transportation, and proof of automobile insurance.
