Job Description
MCHC's Dora Street Health Center is seeking an full-time, HIV Nurse Case Manager. The HIV Nurse Case Manager provides excellent service and access to care for all patients with HIV/AIDS who are served at MCHC. He/she coordinates an interdisciplinary approach to providing needed primary care services, as well as community coordination for our patients with HIV/AIDS.
Duties Include:
- Receives and responds to all telephone contact from patients and/or community agencies and providers for our HIV/AIDS population.
- Participates and assists in implementation of HIV clinical care guidelines.
- Provides the coordination of all primary care including care by community agencies, keeps track of visits, enters them into the database in compliance with our Ryan White Part C Work Plan.
- Provides medication adherence counseling, education, refill information and the monitoring of all patients and their medications. This includes working with the patient, pharmacies and the insurance companies to provide continuity of medication availability and collaborating with HIV medical providers to do medication reconciliation.
- Monitors lab work and provider visits in compliance with the Ryan White Program Work Plan.
- Makes appropriate referrals to community resources. (e.g. patient’s financial concerns, difficult social situations, difficult placement issues, and other multi-disciplinary referrals.)
- Previews and monitors provider PAQs in EMR for incoming patient information which may need to be communicated to part time providers in a timely manner.
- Along with the Program Director, monitors and approves the use of any Ryan White funding requests in accordance with the Ryan White grant award.
- Maintains and provides data reflecting the identified Quality Measures and participates in quarterly and annual data reporting.
- Assists with implementation of care plans, based on mutual goal setting and with emphasis on client decision making. Client education implemented with client response documented on charts checked.
- Monitors client adherence and compliance with medical regimens and clinical outcome measures.
- Acts effectively and in a positive manner as a liaison between the Clinic and other community agencies in mattes relevant to client care.

Minimum Qualifications:
- Active CA LVN licensure
- Minimum of three years’ experience as a nurse, with one year in community-based nursing
- Current CPR certificate Have and maintain a valid California Driver’s License and current automobile insurance policy (liability/bodily injury on his/her automobile(s) used for work)
Preferred Qualifications:
- RN license with strong clinical background, BSN & PHN
- Knowledge of current HIV medical protocols and disease processes
