This position requires you to work on site at one of our facilities located throughout Broward County.

DESCRIPTION OF DUTIES AND RESPONSIBILITIES
This is a paraprofessional position that reports directly to a Linkage and Retention Supervisor. This position is responsible for client intake, screening, and referral of persons referred under the Test and Treat initiative who require linkage to HIV primary care and health support services. Provides referrals to HIV-negative persons at highest risk of acquiring HIV (including those in sero-discordant relationships) to biomedical intervention services, such as PrEP (Pre-exposure prophylaxis) and nPEP (Post-exposure prophylaxis).
Conduct intake interviews and administer program screening instruments to identify range of client needs, barriers to care, and draft a care plan for clients. Maintain good relationships with all community service providers & stake holders. Ensure that clients are linked to community service providers within the established protocols. Enters data information on all clients that are currently being linked to care in the database to ensure continuity of care. The data entered must be accurate and monthly reports are generated and provided to the Linkage to Care Supervisors.
Responds to phone calls from internal and external clients and customers requesting information about Linkage to Care Services and arrange intake appointments when appropriate. Coordinate client care with providers by telephone, as appropriate. Maintain client documentation in accordance with standard of care. Maintain accurate and confidential case records on services provided per Broward County Health Department procedures.
Attend all appropriate meetings as required. Keep the program supervisor apprised of activities, challenges, and needs for assistance.
Perform other related duties as required
MINIMUM QUALIFICATIONS :
•Do you have experience doing case management, peer support, linkage, or retention to clients with HIV to medical or support services?
•Do you have a valid driver's license?
•Do you have experience conducting interviews?
•Do you have experience working with medical records?
PREFERED QUALIFICATIONS:
•Do you have experience using Provide Enterprise?
•Do you have experience conducting field or home visits?
