DUTIES & ESSENTIAL JOB FUNCTIONS: NOTE: Following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.
1. Able to perform all duties of a HIM Technician I.
2. EHR analysis of physician documentation IAW Joint Commission, Title 22 and CMS.
3. Joint Commission Open Chart Review audits.
4. Maintain accurate tracking of paper medical record to include performing medical record destruction in accordance with policies and procedures.
5. Maintain integrity of the EHR by assuring incomplete medical records are appropriately analyzed and assigned to the physician for completion.
6. Monitors incoming documents daily and provides timely completion of HIM functions according to policy; maintains strictest confidentiality of patient information; observes the privacy needs of patients at all times.
7. Performs quality control reviews and remediation of scanned data into the Electronic Health Record (EHR) System, within established policies and procedures for entering documentation into the EHR System; effectively prioritizes workflow to ensure that all patient data is entered within specified turnaround times.
8. Performs Vital Statistics responsibilities to include birth and death registry processing.
9. Processes release of information requests, subpoenas, and other requests consistent with all applicable regulations and policies Serves as a liaison between Health Information Management and other departments as needed.
10. Provides accurate data and reporting for physician suspension process.
11. Researches patient demographic information to ensure correct match to the electronic record; accesses patient demographics in EHR system by name date of birth or social security number, as needed to verify patient identification.
12. Reviews scanned documents for accuracy and makes corrections with overall accuracy expected.
13. Reviews/researches EHR to ensure no duplication of records occurs. Ensures that ordered tests have been properly interfaced and are included into the EHR System.
MINIMUM QUALIFICATIONS
Education: Associate's Degree or Equivalent (may be replaced by three years acute care HIM Experience).
Minimum Experience: Recent three months acute care hospital HIM/electronic health record experience to include analysis, regulatory monitoring, release of information, vital statistics, scanning, indexing and data analysis and reporting in an EHR environment.
Preferred Education: Bachelor's Degree in Healthcare, Business or Related Field.
Preferred License/Certification: Registered Health Information Technician (RHIT).
Required Demonstrated Proficiency: Basic Computer Skills -passing score of 80%.
Required Demonstrated Proficiency: Medical Terminology -passing score of 80%.
Required Demonstrated Proficiency: Typing/Data Entry -40 words per minute.
