Responsibilities:
- Provide personal care to patients in their homes, including bathing, dressing, and grooming.
- Assist with mobility and transfers, ensuring the safety and well-being of patients.
- Monitor and record vital signs, such as blood pressure and temperature.
- Assist with tube feeding and other specialized care for patients with specific medical needs.
- Provide companionship and emotional support to patients.
- Help with light housekeeping tasks, such as cleaning and organizing.
- Collaborate with healthcare professionals to develop and implement care plans.
- Maintain accurate and up-to-date documentation of patient care activities.
Requirements:
- Valid certification as a Home Health Aide or equivalent.
- Experience in providing care for individuals with dementia, developmental disabilities, or hospice care is preferred.
- Knowledge of medication administration protocols and procedures.
- Ability to lift and transfer patients safely.
- Excellent communication skills and the ability to build rapport with patients and their families.
- Compassionate and empathetic nature, with a genuine desire to help others.
- Strong organizational skills and attention to detail.
- Ability to work independently and follow instructions from healthcare professionals.
- Familiarity with assisted living facilities is a plus.
