How You'll Make a Difference:
Arkos Health is seeking a Healthcare Outreach Scheduler. The Healthcare Outreach Scheduler will be responsible for conducting outreach to our specific member population to encourage healthy behaviors through innovative telephonic communications. Performance driven on all provided targeted lists received by our Health Care partners. The Quality Care Coordinator will also enter data into our data base for all CPT II codes being performed. The Quality Care Coordinator will schedule and follow up on clinical care needs for patients as it relates to Healthcare Effectiveness Data and Information Set (HEDIS) and other quality STAR initiatives. The position contributes to the organization's drive to improve the overall experience of patient care, including the quality of health, and work to reduce the cost of care across the population served.
Essential Duties and Responsibilities:
- This position is responsible for reviewing data collected to confirm gaps in care are captured
- Coordinate and order preventative and diagnostic tests around gaps in care on behalf of the primary care physician utilizing standing orders/protocols required by the health plan
- Conduct data entry and maintain appropriate documentation and tracking for quality initiatives
- Conducts outreach to specified patients on priority lists for any outstanding care gaps and schedule accordingly
- Educating patients on procedures that are being scheduled/performed and the reason for performing them
Qualifications and Skills:
- High school diploma or GED equivalent required
- Medical Assistant (MA) Certification and or Certified Nurse Assistant (CNA) Certification preferred
- Current CPR required
- 2+ years' experience in a healthcare environment
- Recent and related experience working with HEDIS STARS Program
- Experience with Microsoft Word, Excel, and Power Point
- Experience in Electronic Health Records
- Ability to collect and process specimen samples
- Experience with verifying Eligibility and navigating through Insurance Portals
- Experience with CPT II codes
- Basic knowledge HCC’s and Medical Diagnosis
- Familiarity of Medicare guidelines to perform patient screening’s
- Medical Terminology
