The Director of Health and Wellness is responsible for the coordination of care for all Liana North Sarasota residents, including case management and assuring appropriation of care. The Director of Health and Wellness is accountable for all planning, organizing, directing and evaluating of the Nursing Department.
Qualifications
· Must be a graduate of an accredited school of nursing and must have and maintain a valid, unrestricted nursing license in the state.
· Minimum of three (3) years successful leadership role in a community or long-term care setting.
· Experience with aging adults and knowledge of regulations pertaining to Assisted Living Facilities/Memory Care preferred.
· Individual must also be able to read, write, and speak the English language and be able to communicate effectively with residents, family, physicians and staff.
· Ability to react calmly and confidently under pressure and in unexpected situations.
· Make independent and good judgment decisions within proper policy and procedures.
Essential Functions
1. Oversee the recruitment, hiring, training, supervision, and evaluation of all care associates to assure quality services.
2. Oversee the planning and delegation of work assignments, ensuring effective scheduling.
3. Provide case management services to residents, ensuring completion of the pre- occupancy screening, assessment/reassessment, and the service plan in conjunction with the resident’s family and other staff.
4. Expands personal development, skills and knowledge of activity programs as it relates to older adults with moderate to severe memory impairments in a long-term care setting.
5. Facilitate and encourage family involvement with residents.
6. Assist in coordination of resident move-in and orientation of new residents to the community. Assist in the coordination of move-out, ensuring a smooth transition for the resident and family.
7. Investigate resident and associate incidents, identify safety hazards, and institute corrective actions in a timely manner.
8. Develop and monitor resident service plans, record information to determine quality of service to residents, and direct staff to implement needed changes.
9. Monitor and maintain records reflecting resident’s physical and mental condition. Report significant changes in condition to the Executive Director and physician.
10. Coordinate wellness activities and participate in the resident activity program.
11. Participate in developing and implementing the Community Marketing Plan.
12. Ensure compliance with regulatory agencies. Participate in facility surveys initiated by government agencies and/or management.
13. Operate department within budgetary guidelines. Work with the Executive Director to make recommendations for capital expenditures.
14. Serve as Manager on Duty (“MOD”) as assigned.
15. Work with other members of the management team to market and manage the community.
16. Attend and participate in in-service educational classes and on-the-job training programs, etc., as scheduled or directed.
17. Perform all other duties as assigned.
Job Types: Full-time, Part-time
Pay: $70,000.00 - $80,000.00 per year
