Under direct supervision, responsible for ensuring the quality of Medical Records by verifying for completeness, accuracy, and proper entry into computer systems. Assembles and analyzes patient data for the purpose of improving patient care or controlling costs.
Job Duties:
- Ensures specialization in coding diagnoses and procedures in patient records for reimbursement and research.
- Analyzes patient Medical Records received from patient care areas to verify for completeness and accuracy.
- Conducts quality checks on order entry, document imaging and clinical documentation in order to instruct/train end users in the appropriate method of entry/selection/linking orders to appropriate document levels within the management system(s).
- Coordinates with physicians and patients to complete records containing deficiencies.
- Conducts daily review of overdue results report for outstanding lab/imaging/procedure results and retrieves from the appropriate facility.
- Performs document-imaging processing for all retrieved lab/imaging/procedure results and finalizes the overdue result in the electronic medical record.
- Ensures all records are properly coded/indexed into the appropriate management system(s).
- Serves as a liaison to facilitate accurate Health Information.
- Performs all other duties as assigned.
Education:
High school diploma or GED is required.
Experience:
Five (5) years Medical Records or relevant clerical experience is required.
