HEALTH IMPROVEMENT SPECIALIST - SPANISH OR SOMALI SPEAKING
Work Location: Office for Your Day
Position Description
As a Health Improvement Specialist - Quality, you will be responsible to provide culturally congruent member outreach to identified members to improve and support members and programs including regulatory reporting, population health strategy, access to benefits, HEDIS rates, DHS withhold measures, NCQA Accreditation and Star measures for all products. This position will also support efforts to provide health education to members and assist with setting up medical appointments, transportation, interpreter services, and related services in effort to close gaps in care and increase access to benefits and resources. Provide general education to members about preventive health plan benefits available to them and support efforts to meet NCQA, CMS and DHS guidelines.
• Provide culturally congruent outbound telephonic engagement calls and provide health education to members who meet criteria for population health programs, HEDIS, DHS withhold measures, and Star measures.
• Document call outcomes in UCare's case management system, schedule medical appointments and necessary services, including support services for members in obtaining preventive care and services, taking into account the member's unique needs, and working to facilitate culturally congruent services whenever possible.
• Connect members to community resources congruent with their needs identified in the population health strategy or when conducting telephonic outreach. Collaborate with members and/or family members, primary care physicians, clinic staff, providers and other relevant agencies to assure appropriateness of services that meet member needs and ensure desired outcomes.
• Enter members who wish to opt-out of further calls/mailings, etc. into the 'Opt-Out' list.
• Engage in development and implementation of member facing interventions (e.g. member materials - mailings, emails IVR calls/scripts, newsletter articles, etc.).
• Utilize UCare systems to send member mailings regarding rewards, supplemental benefits, and/or appointment reminders in the mail.
• Provide feedback on best practices regarding written and verbal engagement with members to leadership to help support member experience and interventions.
• Complete productivity tracking document as directed.
• Collaborate with teams to document, schedule and identify project timelines, milestones, resource allocation and problem-solving strategies.
• Collaborate to develop and implement strategies to improve population health goals, HEDIS rates, DHS withhold measures, NCQA Accreditation Ratings, and CMS Star ratings.
• Maintain good working relationships and open communication with internal and external customers and stakeholders.
• Other projects and duties as assigned.
Education
Bachelor's Degree, Community Health Worker Certification, AA degree or two years' experience in human services, public health, community outreach, or community health-base setting.
Required Experience
Two years of experience in a health or medical care setting such as a public health department, physician group practice, health plan, hospital, or community clinic. Proficient PC skills and Microsoft Office (with emphasis on Excel spreadsheets).
Preferred Experience
Bilingual in Spanish, Hmong, Somali, or Karen. Knowledge of and experience in telephonic outreach coordination. Excellent computer word processing and database experience. Experience working with diverse populations. Experience building relationship with tribal nations and/or other community-based organizations.
