This position is fully remote but will require travel to client locations (10% max). We are only accepting applications for local candidate able to travel to the Philadelphia and Harrisburg areas as needed. We thank you for your understanding.


In this position you will be supported by clinical teams in your strengthening of the connection between the patient and their primary care physician/medical practice staff, as well as improving the patient’s health and well-being. This role involves engaging with patients and helping them navigate their care, solve their healthcare issues, and improve communication with their medical practice.


WHAT YOU’LL BE DOING

  • Working remotely with the patient’s medical practice staff to manage a targeted population of impactable patients.
  • Functioning independently as well as being a highly collaborative team member as an extension of medical practice.
  • Assisting the patient in achieving better health by addressing their questions and concerns, connecting them with the medical practice, assisting in developing high-impact care plans and closing care gaps, escalating medical issues, and navigating them to the right resources including assisting with referrals to specialty care and community-based organizations (CBOs).
  • Accurately documenting patients’ medical history and medical issues in the electronic health record (EHR).
  • Understanding the referral and prior authorization process.
  • Continuously building a trusting, professional, working relationship with assigned medical practices.
  • Learning how to use effective communication modalities (calls, texts, portals, emails) to engage impactable, high-risk members.
  • Appropriately escalating medical issues to the Healthguide Registered Nurse and to the medical practice.
  • Traveling to the supported primary care physician offices a couple times a year or as needed.

WHAT YOU'LL NEED TO HAVE

  • Extensive experience and understanding of the function of a primary care physician practice.
  • Experience in behavioral health and/or population health and disparities.
  • Strong knowledge of social determinates of health
  • Bilingual (English and Spanish) strongly desired
  • Certified Medical Assistant (preferred) or equal experience
  • Certified Community Health Assistant (preferred) or equal experience
  • Must have a cheerful outlook and empathy for the patient.
  • Comfortable learning and utilizing multiple different computer software programs.
  • Ability to multi-task and balance competing priorities.
  • Familiar with a wide variety of electronic health records, such as: EPIC, Athena, Cerner.
  • Exceptional conversation skills while multitasking by documenting patients interview software in real time.
  • Exceptional written and verbal communication skills.
  • Ability to track the progress of multiple tasks while ensuring that tasks are completed.
  • Ability to work independently as part of a patient’s overall care team.

Guidehealth’s compensation structure includes more than just base salary. Our paid time off program includes paid holidays and flexible time that focuses on meeting the demands of the position and our clients, while providing the balance that our employees need.


The base pay range for this role is between $20 to $23 per hour paid bi-weekly per our standard payroll practices. Final base pay decisions are dependent upon a variety of factors which may include, but are not limited to: skill set, years of relevant experience, education, location, and licensure/certifications.
