Job description

Position Summary:
The Department of Research & Health Promotion augments SYHealth's clinical care with health promotion, research and care coordination programs for patients and community members. Under the supervision of the Health Support Services Program Supervisor, the Health Educator will be responsible for providing one-on-one and group-based health education, in both English and Spanish. The Health Educator will be based in Alpine, with additional outreach within the wider East San Diego region. Health education sessions will be for adult patients, covering topics such as nutrition, physical activity, stress management, and chronic disease prevention and management. Health education sessions include clinic-based, telehealth, and community-based sessions as required. General responsibilities will include, but are not limited to: contacting potential participants, delivering one-on-one and/or group health education, conducting outreach and community presentations, and other health education duties as directed.
Essential Functions of the Job:
  • Provide one-on-one and/or group-based telehealth and/or in-person health education programming as needed to patients, including evidence-based and other programs as directed by Health Education Program Supervisor.
  • Contribute to the development and implementation of educational curricula/materials for use in clinics and to distribute in the community.
  • Recruit patients/participants, and conduct clinic in-reach activities as needed to facilitate access to health education services and resources.
  • Act as primary contact for SYH clinics, schools, family resource centers and community based organizations requesting health education information.
  • Schedule and conduct community presentations and outreach activities that will contribute to the promotion of a healthy lifestyle and the Health Education Department, as requested.
  • Be responsible for keeping tracking logs and conducting data entry for participants receiving health education services, including one-on-one, group, presentations and outreach activities.
  • Attend Community Meetings as requested by Supervisor.
  • Report to and meet with Supervisor as needed.
  • Serve as a liaison between the community and the health center.
  • Conduct activities that will contribute to the promotion of healthy lifestyles.
  • Conducts self in a professional and positive manner in all clinic and community related activities.

Additional Duties and Responsibilities:
  • Maintains established departmental policies and procedures, objectives, quality assurance program, safety, environmental and infection control standards.
  • Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
  • Maintains program specific records as required; submit reports, including activity, contact and mileage reports in accordance with agency policies and procedures.
  • Attends meetings and/or training sessions as required and participates on committees as directed.
  • Performs other related duties as assigned or requested.

Job Requirements
Education and Experience Required:
High School Diploma or GED equivalent and minimum of 2 years of experience providing health education in group and/or individual settings; or any combination of training and experience which would provide the required knowledge, skills, and abilities. Examples of qualifying experience include:
  • Graduation from an accredited four-year college or university with a Bachelor's degree in a health related field and 1 year of health education experience.
  • 2 years in a position comparable to Health Educator in a community, clinic, or school-based setting.
  • Other combinations of education and experience may be considered.

Education and Experience Preferred:
Bachelor's Degree in Health Education, Social Work, Psychology or related field; experience providing one-on-one health education and delivering presentations in a group setting.
Certifications Required:
  • Valid CA Driver's license and insurance - required
  • Regular & reliable access to personal vehicle

Verbal and Written Skills Required to Perform the Job:
  • Excellent interpersonal skills.
  • Excellent verbal and written communication skills.
  • Bilingual: English/Spanish (speaking/writing/reading) - required

Technical Knowledge and Skills Required to Perform the Job:
  • Familiarity with and knowledge of community resources and their use to meet the needs of families.
  • Ability to follow instruction and excellent problem-solving skills.
  • Capability to establish and maintain good interpersonal relationships.
  • Ability to work independently and meet deadlines.
  • Must maintain confidentiality at all levels.

Equipment Used:
Computers, phones, copy machines, fax machines, Microsoft Office (Word, Excel, and Outlook)
Working Conditions and Physical Requirements:
Prolonged sitting position. Frequent walking and standing at outreach events. May be required to work evening and weekend hours. Be able to carry a minimum of 20 pounds. Traveling within East San Diego is expected in this role.
