JOB SUMMARY: Under the Program Manager's direct supervision, the Health Educator provides the health relationship curriculum and runs the Peer Leadership Program. Additionally, the Health Educator is responsible for program recruitment, group facilitation, conducting intake/ behavioral assessments to examine risk factors and make appropriate referrals as needed.
DUTIES AND RESPONSIBILITIES:
- Deliver Healthy Relationship curriculum 20 HIV+ clients with 100 encounters. Conduct all phases of the program curriculum as outlined.
- Peer leadership Program to the target population 2 cycles of 10 weeks of 4 peers each for a total of 160 encounters.
- Create linkages with agencies that service the HIV+ population such as HASA, medical offices, SROs and shelters, and other agencies to recruit the target population Maintain open communication with the referring sources as required on the progress the participant is making in the program
- Participate in 12 events at BAC Each event must have a minimum of 45 people
- Attend and participate in 9 events outside of BAC (Including some weekends) Each event must have a minimum of 45 people
- Conduct all services and satisfy all deliverables as stipulated in the program contract.
- Conduct Behavioral Assessment of clients’ health risk and HIV status to determine client’s risk of acquisition of HIV and STD/HCV
- Make referrals if necessary to other BAC providers to address psychosocial needs
- Attend training and conferences pertaining to the program and target audience.
- Document all services in 24 hours in accordance with BAC recordkeeping policies.
- Other duties as assigned based on program needs.
KNOWLEDGE, SKILLS, AND ABILITIES:
- MINIMUM: Two (2) years in group facilitation, or providing education in a similar Exceptional written and verbal communication skills, including effective public speaking and interpersonal skills
- Ability to collaborate with internal and external stakeholders
- Proficient in Microsoft Office (Word, Outlook, Excel, PowerPoint, etc.)
- Previous experience in substance use prevention Motivational Interviewing, Harm Reduction and Trauma-Informed Care approach, preferred
- Ability to withstand extended sitting, extended standing, stairs, and extended travel with or without reasonable accommodations.
- Ability to learn computer databases eCR
- Ability to meet deadlines, multi-task, prioritize, think logically, follow instructions and work in a fast-paced environment.
- Must be willing to work weekends, nights, and long/flexible hours as required by the position.
EDUCATION
- MINIMUM: High School Diploma/ GED with at least three (3) years of professional experience in the field or
- Bachelor’s Degree in health education or related field and one (1) year of relevant experience
WORK EXPERIENCES:
- MINIMUM: Two (2) years’ experience working with the population of focus
- Strong outreach Skills, including creating partnerships,
- Experience in group facilitation, comfortable with discussion/teaching classes.
- Familiar with using EHR systems, plus
- Effective communication, including writing, speaking, active listening, and presenting
- Up to 50% of traveling required.
LICENSURE: A NYS Driver’s License in good standing, preferred.
