Location: PH&W HQ - 400 E. Gray St
Healthy Start
Location: Remote, main work location would be Bridges of Hope, 1411 Algonquin Pkwy, Louisville, KY 40210

Summary Objective
The purpose of this position is to plan, develop, organize and evaluate Health Education program exhibits, literature and audiovisual materials for the agency.
This class works independently, under limited supervision, reporting major activities through periodic meetings.

Examples of Duties

ESSENTIAL FUNCTIONS

The essential functions listed below are those that represent the majority of the time spent working in this class. Management may assign additional functions related to the type of work of the class as necessary.

  • Develops and prepares Health Education materials and programs.
  • Conducts Health Education and training programs at various sites and to specified assigned populations.
  • Research information and provides consultation to professionals on health-related topics.
  • Plans, develops, organizes, and evaluates Health Education program exhibits, literature, and audiovisual materials for the agency.
  • Performs scientific and Health Education research.
  • Conducts and directs Health Education programs, training programs, lectures and classes for new employees, volunteers and community groups, and transports equipment and training materials to various training sites.
  • Prepares literature and information for distribution to the media, community organizations and the public.
  • Recommends revisions to existing programs and develops new programs in consultation with department authorities.
  • Maintains the agency's resource library of books, publications and audiovisual materials.
  • Represents the department to community groups and other governmental agencies to provide information and answer questions concerning public health programs and services.
  • Actively participates in networks and coalitions.
  • Develops and implements outreach and educational initiatives to address emerging trends/needs Collaborates with community partners to ensure Louisville residents and visitors are linked to care and resources and/or to ensure awareness of Division activities and collect feedback on service delivery to improve programs.
  • Prepares and submits reports, research, presentations, and other materials for Division as designated by supervisor.
  • Performs related work as assigned.

HARM REDUCTION OUTREACH SERVICES:

  • Represents the department to community groups and other governmental agencies to provide information and answer questions concerning public health programs and services.
  • Conducts and directs Health Education programs, training programs, lectures and classes for program participants, community members, external organizations, new employees, and volunteers, and transports equipment and training materials to various training sites.
  • Plans, develops, organizes, and evaluates Health Education program exhibits, literature and audiovisual materials for the agency including but not limited to Harm Reduction Strategies, HIV prevention, Sexual Health information and Community Referral Services.
  • Deliver Harm Reduction Services and education to participant base and community.
  • Connects people in the community to harm reduction, treatment, and other supportive services.
  • Engages in ongoing continuing education opportunities to maintain evidence-based knowledge and practice to ensure programmatic goals and outcomes are met.
  • Engages in community outreach to provide education, resources and create opportunities for relationship building with identified populations and community members in general.
HEALTHY START FATHERHOOD PROGRAM:
  • Conducts research to create and implement curriculum and educational materials for use with Louisville Metro Dept. of Corrections (LMDC) residents (inmates) who are fathers.
  • Provide case management for those that qualify for healthy start services in the community and once released from LMDC in an effort to help reduce recidivism.
  • Stay on top of current research and trends related to fatherhood and men's health.
  • Creates and updates as needed curriculum for new classes for all healthy start fatherhood participants.
  • Represents the department to community groups and other governmental agencies to provide information sharing and awareness and advancement of Healthy Start projects fatherhood program.
  • Works with LMDC and serves as chair of fatherhood subcommittee for Healthy Babies Louisville.
  • Creates opportunities and a safe environment for fatherhood participants to engage in their own health, as well as their child's health before, during, after and beyond pregnancy.
  • Assesses the impact of father involvement on maternal health and birth outcomes to identify opportunities to enhance and strengthen projects and strengthen family resilience.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS
  • Bachelor’s degree in Education, Public Health, Public Administration, Environmental Sciences, Human Resources, Sociology, Business, Human/Social Services, Social Sciences or related discipline.
  • Two (2) years of conducting Health Education and training programs experience or an equivalent combination of education, training and experience.

Minimum Requirements

PHYSICAL DEMANDS

Physical demands refer to the requirements for physical exertion and coordination of limb and body movement.

  • Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.
  • Meets regular and punctual attendance standards and any special needs of the position as determined by management, including mandatory overtime, and performing related work.
  • Travels to various locations including local health organizations, schools, and other venues to reach target groups/population.
  • Must wear personal protective equipment as mandated by OSHA
  • Must be vaccinated for hepatitis B, have provided evidence of immunity, or have signed a declination form within ten days of initial assignment.
  • Must provide one's own source of transportation.
  • Must work a varied schedule.
  • Must possess and maintain a valid state driver’s license with an acceptable driving history.
HEALTHY START FATHERHOOD PROGRAM:
Travels to various locations including LMDC, community partners, and other venues to reach target population.

HARM REDUCTION OUTREACH SERVICES:

  • Based at Public Health & Wellness Department Headquarters in office setting.
  • Driver’s license an asset.
  • Ability to lift up to 40 lbs. an asset.

SKILLS AND ABILITIES:

Ability to maintain confidential information.

Ability to effectively work with diverse population.

Proficient in the art of verbal and written communication skills

Must be detail-oriented and able to prioritize tasks.

SPECIAL CERTIFICATIONS AND LICENSES

HARM REDUCTION OUTREACH SERVICES:

  • Familiarity and comfort with the principles of harm reduction highly desired.
  • Lived experience of substance use disorder is highly desired.
  • Outreach experience and public education experience preferred.
